PhilHealth requires ID for issuance of MDR
Cebu City (28 November) -- Philippine Health Insurance Corporation (PhilHealth) requires all members to present identification card or other appropriate documents to verify the identity of the person requesting a member data record (MDR).
The MDR is a primary document in filing claim benefits since September 1, 2007 as per PhilHealth Circular No. 7, series of 2007 as amended by Circular No. 12.
Members requesting for MDR must present a request form or duly accomplished Claim Form 1 together with the PhilHealth card (employed and individually-paying/voluntary members), Family Health card (sponsored/indigent members). Lifetime or non-paying members must present their ID. If the PhilHealth card/ID is not available, the member may present any valid ID.
Authorized representatives requesting for MDR must present an authorization letter signed by the member or a duly accomplished Claim Form 1, together with a valid ID of the member and the said representative.
The nearest kin must present a letter of request if the member cannot provide an authorization letter. The request must be presented by the kin together with a valid ID and a document to prove one's relationship with the member.
PhilHealth only issues the MDR to members who are already registered or have their PhilHealth identification numbers. (PIA7/hfg-phic) [top]
