MDR required in availment of Philhealth benefits

San Fernando, Pampanga (18 September) -- Effective September 1, the Philippine Health Insurance Corporation (Philhealth) requires the submission of a Member Data Record (MDR) to avail oneself of his/her Philhealth benefits.
This was announced by the agency's information office as they described the MDR as a "computer-generated report enclosed with the Philhealth number card given to a member".
Philhealth recently issued Circular No. 7 and 12 requiring all members - whether they are employees, self-employed, non-paying members, overseas Filipino workers (OFWs) and GMA card holders - to submit their MDR alongside their accomplished Philhealth Form 1 when availing themselves of hospitalization benefits.
In a press statement issued by the agency, Philhealth said the MDR contains member information and the names of his/her dependents and added that submission of MDR would facilitate processing and avoid "return to sender claims".
In case a member's MDR is lost or destroyed, he or she could request a copy from any Philhealth Regional or Service Office in his/her area. For any discrepancies on the information contained in the MDR and the Philhealth claim Form 1, corresponding documents such as birth certificate or marriage contract may be presented as proof of dependency, along with the Member Data Amendment Form or M2 which can also be gotten from Philhealth offices.
Philhealth also called on those who do not have Philhealth numbers but use their SSS numbers in paying their Philhealth premium to go to the nearest regional or service office to get their numbers and MDRs.
